Incident Report Forms

 

General Incident Report Form 

To be used to submit information related to general incidents. This may include general violations of our Student Code of Conduct or other incidents/concerns that occurred on or off campus. 

Student of Concern Reporting Form

To be used to refer a student to the Geneseo CAREs Team. This includes concerns about a student(s) well-being, mental/physical health concerns, or other instances where students may benefit from being connected to campus resources. 

Title IX Incident Report Form

To be used to report incidents involving students and/or staff that fall under the purview of the Title IX Office. 

Bias-Related Incident Report Form

To be used to report incidents or concerns that fall under the purview of the Office of Diversity and Equity.

Hazing Incident Report Form

To be used to report concerns or incidents of potential hazing activities. Please note that the organization of concern does not necessarily need to be “registered” with the College to submit your concerns. 

Notification of Arrest

To be used to self-report incidents where students are arrested for a misdemeanor or felony arrest. 

Petition for Transcript Notation Removal

To be used to submit a request to remove a notation placed on an academic transcript as a result of a student conduct matter.
