Health Requirements - New Incoming Students

Immunization Records and Requirements

Student Health and Counseling will obtain your immunization records from the NYS Database.

  • If your records are missing or incomplete, we will contact you for more information.
  • Students not from New York State or from a different country will need to upload their Immunization Record to their student My Health portal under Medical Clearances.
  • Students must meet the New York State Measles, Mumps, and Rubella (MMR) and Meningitis Immunization requirements within 30 days of the start of their semester. Those who do not meet these requirements will be disenrolled for non-compliance.

These requirements apply to students taking 6 or more credits per semester.

TB Screening

This form appears as an online questionnaire on the My Health portal.

MMR (Measles, Mumps, Rubella)

New York State requires students taking 6 or more credits to have been vaccinated against MMR with 2 doses of vaccine given at least 28 days apart or a blood test showing immunity.

Meningitis/Meningococcal

Students are required to have a MenACWY within the last five years from the start of the semester date. This vaccine may appear on your records under various names; review the CDC's Vaccines for Meningitis page for more information. Students may receive the vaccine at any time in the future through their healthcare provider or at a local pharmacy. Students are also encouraged to be vaccinated against meningitis B before arrival to campus but this is not required.

Exemptions

New York state law allows a student to request a waiver from a vaccination required by law. Conditions for waiver: 1) the student has a medical reason to decline immunization, or 2) the student holds genuine religious beliefs which are contrary to the practice of immunization. Forms are completed by the student or by the parent/guardian for those under 18 years of age.

For MMR

There are two types of waivers for MMR:

  1. Medical Exemption 
    This form must be completed by a licensed physician, nurse practitioner, or licensed midwife (for a pregnant student) who is caring for the student. This form certifies that the student has a health condition that prevents receiving the MMR vaccine.
  2. Religious Exemption 
    This form is completed when student holds genuine beliefs which are contrary to the practice of immunization. Requests for religious exemption are to be submitted with a written statement from the student.

For Meningitis/Meningococcal

To complete a waiver for Meningitis/Meningococcal:

  1. Login to the student health portal, My Health, using your SUNY Geneseo email and birth date.
  2. Select Medical Clearances from the MENU.
  3. Scroll down to "Items NOT Required for Clearance" and select Meningococcal Waiver.

Note that in the event of an outbreak, medical or religiously exempt students may be asked to leave the SUNY Geneseo campus for their own safety.

Additional Suggested Immunizations

There are a number of other potentially life saving vaccines that are not required by law, including influenza, hepatitis B, HPV and others. These should be reviewed with your PCP prior to coming to campus or an appointment can be made with the health center to discuss how to be immunized.

NOTE: The COVID-19 vaccine is no longer required for students to attend SUNY Geneseo.

Physical Exam

Components of Recent Physical Requirements

Students should submit a physical from within the last 18 months with:
   1) Student's Name and Date of Birth. 
   2) Date of examination
   3) Vital signs including blood pressure
   4) Provider's name, contact information and signature
   5) Statement that the student is healthy to attend college and/or play collegiate sports

You may also download our physical form for your provider to complete.

Varsity athletes require a physical within 6 months of the start of your sport.

NOTE: a visit summary from your providers' health portal will NOT be accepted as a valid physical.

Consent for Medical Treatment of a Minor

If you are under 18 years old when you arrive on campus, your parent/guardian must complete the a Consent for Medical Treatment of a Minor.

Health History

On the Health History Online Form, you will enter your allergies, medications, personal health history as well as family history.

Insurance Card

You must upload your insurance card to your student health portal, My Health.  

  1. Log into My Health using your SUNY Geneseo email and birth date.
  2. From the main menu, select Insurance Card.
  3. Chose the button for Add New Card.
  4. Scroll to the bottom of the page that comes up.
    • Click the Upload Front Image button. Select the front image of your insurance card then chose DONE.
    • Click the green button LOOKS GOOD to confirm.
    • Repeat this process for the back of your card.
  5. Select the button Populate Form (OCR). This will auto-populate the fields.
  6. To finish, select SAVE.

You MUST contact your insurance company BEFORE arriving to verify that our lab, Quest-Pittsburgh 875 Greentree Road, Bldg. 4, Pittsburgh, PA 15220, is in your insurance network and whether or not you are required to obtain prior authorization for labs. If they are not in your network, labs can be billed to your student account for payment. When you visit Student Health and Counseling, you will be responsible for any charges your health insurance does not cover.
