Clinical Case Management

Clinical Case Management aims to provide short-term and goal-oriented interventions for students with concerns in singular or multiple life areas. Clinical Case Management directly supports students in overcoming obstacles, reducing distress, and connecting to appropriate on- and off-campus resources by meeting with students as an included service from Student Health and Counseling, located within the Lauderdale Health Center. 

The scope of Clinical Case Management includes:

  • Helping students identify basic need areas, personal goals, and barriers to resources
  • Identifying paths for students to access and expand their support network
  • Offering an empathic perspective and guidance on utilizing their support network to manage stressors.
  • Communicating with campus partners and off-campus resources to advocate for the student and provide supportive collateral information.
  • Reviewing Student of Concern reports and managing cases referred to/by Geneseo CAREs. 

How to Schedule:

Students can directly request a case management appointment online via the student health portal, MyHealth (for step-by-step instructions, visit Using the My Health Portal). As with all on-campus offices and resources, faculty and staff are encouraged to provide Clinical Case Management information to students whom they have identified as “in-need” of this intervention.

What to Expect: 

Upon requesting a case management appointment, our Clinical Case Manager, Rachel Radovich-Chu, LMHC will reach out to schedule via secure message on the student health portal. Depending on your circumstances, you may have one or multiple appointments throughout the semester; however all students will have an initial appointment to assess wellness, areas of need, and determine best steps going forward. Below are some examples of case management assistance provided to students:

  • Connecting with Financial Aid to organize and complete required paperwork
  • Advocating with students for additional academic resources and support from instructors
  • Guiding students through off-campus referral processes
  • Navigating social services and community agency networks (i.e. Livingston County, LawNY, NYS of Health/Medicaid, etc.)
  • Researching health and wellness resources, and more

As outlined above, the Clinical Case Manager may provide services such as initial assessment of needs, follow-up appointments, referral coordination, consultation and collaboration, and participation on the Geneseo CAREs Team as the CAREs Coordinator or Clinical Case Management representative. Services not within the scope of this office include the following:

  • Ongoing therapy/mental health counseling treatment
  • Diagnostic services
  • Documentation of a disorder, disability, or need for accommodations
  • Therapy animal/emotional support animal documentation
